Order Form
Please list the sticker designs that you would like to purchase and provide your contact information so that we can contact you when your order has shipped. We will only be shipping to addresses within the US.
We will be accepting payment in the form of donation to our Cal Poly Giving. Please submit a donation for the amount of your order total. There is a 10 dollar minimum donation so you must purchase a minimum of 3-4 stickers. Attach your donation conformation email to this form to confirm your order. We will review your donation conformation to confirm that the amount donated matches the order total.
